Home Care Support Worker

We're currently looking for Home Care Workers for a variety of shifts providing personal care and assistance to older people living in their individual homes throughout the Medway and Swale area. We provide day and night time support, a service to enable people to regain their independence and continue to live in the community as long as they wish.

We need people who have a full driving license and are flexible in their availability, able to work early morning hours, evening hours and weekends from 7.00am to 10:00pm.
We are looking for people who have experience of providing support (either from Home Care, residential care or through personal experience) who can work on our enablement service, adhering to individual assessed needs as stipulated service user’s care plan.

Duties to include: 
• Personal care 
• Assisting people with bathing 
• Preparation of meals & light snacks 
• High dependency service 
• Hospital discharge service (where support workers are available to support in the    
home when a service user comes home from hospital) 
• Encouraging and supporting independence and choice
