GETTING YOUR CARE STARTED
When we receive the first telephone call or visit to the office from a Social Worker, family member or friend or indeed you, we then initiate the following process:

  • We will complete a referral form and ask specific questions which will help us understand if we can help you
  • We will send out information to you in the post regarding our service to give you information on what we can do for you this includes the rates we charge
  • Once you have received our information and you are happy to proceed please call us so that we can arrange an appointment for a trained member of staff to visit you and discuss your package of care
  • At this meeting, whereby you can ask for family or a friend to be present we will look at what needs you have and complete an Assessment of Needs form. Some information may be quite personal but this helps us understand your needs and how we can help you
  • Our trained staff will then need to undertake a risk assessment of you, your mobility and also the places within your home where our carer is expected to work (this is in line with Health & Safety Legislation)
  • Once all the necessary paperwork is completed our staff member will then sit with you and go through it to ensure you agree with what has been written. If you agree then we ask that you sign this paperwork to state that you are in agreement.
  • An information folder will be left with you so that you have all the information you need, our staff member will go through this with you and answer any questions you may have.
  • The allocated carer will then relay all this information back to the office team and your care will be planned. We will then contact you to inform you of the carer/s that will be attending you, the days and times
  • The member of staff that visited you in the beginning will ensure that a full service user plan, a copy of the risk assessment and assessment of need. paperwork is delivered and placed into the information folder
  • Your package of care will be reviewed after 4 weeks to ensure everything is running smoothly and adjust where necessary. Thereafter every 6 months unless there has been a change to your needs.

PERSONAL CARE
Your carer will be able to undertake personal care services such as helping you to get up in the morning, washing, dressing, toileting, bed-making, helping you to go to bed at night, etc. The carer can also assist you to take any medicines that have been prescribed for you by your doctor, subject to certain limitations, which will be explained to you.
